1950s–1960s: The Birth of Metabolic Surgery 

The mid-20th century saw the first true bariatric operations: 

  • 1954: The jejunoileal bypass, an early metabolic surgery connecting the proximal jejunum to the distal ileum, induced weight loss but also had serious side effects like diarrhea and malnutrition.
  • 1966: Dr. Edward Mason at the University of Iowa pioneered the first gastric bypass, creating a small stomach pouch and rerouting food to bypass much of the stomach and upper intestine. Later refined into the “Roux-en-Y” technique, this became the gold standard for decades. 
     

1970s–1980s: Safer, Less Invasive Techniques 

Throughout the ’70s and ’80s, surgeons experimented with variants, adding bands to limit pouch expansion and developing gastric banding procedures. In 1980, Mason introduced vertical banded gastroplasty. Then from 1986 to 1993, adjustable gastric bands emerged and became popular for their reversibility. 

1990s: Laparoscopic Revolution 

The 1990s brought a surgical breakthrough. In 1994, Dr. Alan Wittgrove performed the first laparoscopic Roux-en-Y gastric bypass, marking a shift toward minimally invasive, safer, and faster recovery procedures. Minimally invasive sleeve gastrectomy would follow in the 2000s, streamlining care even further.
